The Filter Factory and Transition Factory plugins, which were delivered with Adobe Premiere until Version 5.1, let you easily create your own plugins. Unfortunately Adobe removed them from Premiere 6, but they still work with it.
A harder way is to use the Premiere SDK which can be downloaded from http://partners.adobe.com/asn/developer/sdks.jsp
