Is there a recommended setting for sharpening with Focal Blade when using online photo services for processing such as Shutterfly (understand that is necessary to turn off any automatic enhancement that they do)?

You should always turn off the automatic enhancement feature of online photo services. They often mess up the photos.

You can use the Automatic sharpening features of FocalBlade for this task and activate the Print button.

Would it be better to use medium/heavy sharp, medium/fine details etc?

Well, that depends on how sharp you like it best and the details in the image. I would recommend very fine or fine details and medium sharp plus the Print button activated.
