It may be easier to export movie as FLC/FLI from the export timeline dialogue. This sends it to Photoshop as a "cartoon strip" that can be worked on. Have you tried Premiere's own Video Effects? Go Window/Show Video Effects/Video/Blur/ then drag Camera Blur and drop on the pieces of video you want to affect on the timeline.

It kinda depends on the version of Premiere you're running. The more recent (6.5 up, from memory) allow you to export direct from the timeline. Just go File/Export Timeline/Adobe Mpeg Encoder.

You can do this without a plugin in all versions of Premiere. Right click the piece of video on the timeline, select Speed and adjust the speed percentage.
